Accident and Incident Investigation

It is a fact of life that accidents happen. Even with the best health and safety practises in place, no activity is without risk.

When the unfortunate happens best practise is to learn from the event, understand what has happened and put measures in place to prevent a similar accident or incident occurring in the future.

Accident and Incident investigations should not be restricted to events where a person is killed or seriously injured. By not carrying out investigations into ‘lesser’ events, you will miss opportunities to improve your safety systems and hopefully prevent a more catastrophic incident in the future. A positive safety culture promotes learning from all accidents, incidents and near-misses.

I offer an impartial investigation service which will enable you to get to the root cause of what has gone wrong and identify the learning from the event. Key questions to be answered during the investigation are:

  • What exactly happened and what were the consequences?
  • What were the root causes of the accident?
  • What could have been done differently?
  • Was there any breach of statutory duty?
  • Where does any fault lie (if any)?
  • What lessons can be learnt?
  • What improvements need to be made to prevent a recurrence?
